Once we receive your authorization to access your healthcare
reports and results, we will acquire your reports and results
for you. The healthcare provider is required by law to
provide these reports and results. Just so you understand
that all of your medical information, results, reports
and physician notes are yours and belong to you. It is
up to your healthcare provider how fast they respond to
your request. Your physician or healthcare provider has
the right by law to charge you for theses copies. It is
really up to each provider if they will charge you or
not. The new guidelines indicate that a healthcare provider
can charge up to $1.00 per copied page for the first 25
pages and then $0.25 per page after that. If the caregiver
faxes your medical records, there is usually not
a charge.
PocketMD, LLC. can not be held liable for not being able
to obtain your reports or notes in a timely fashion. We
will make every attempt to acquire your records as soon
as we receive your authorization. We will document the
time we receive your request and the time we actually
requested it from your healthcare provider.
